Executive Summary

The TeleCure Asia team came to us with a vision to end the fragmentation of the healthcare system throughout Southeast Asia. Many patients were experiencing long wait times in the doctor’s office, having trouble communicating with their pharmacist, and not having access to their electronic medical records.

In addition, physicians needed a way to manage their patient population without experiencing administrative burnout.

We set out to create a hybrid version of existing apps (e.g. telemedicine, e-pharmacy, EMR) into one application known as a "super app." We needed to find a way to connect to all of the above technologies while adhering strictly to PDPA regulations.

The end result: an ecosystem of services where over 500 doctors have registered and thousands of consultations have occurred! Our client is now looking to expand into other countries where they will provide the super app services to new customers.

The Core Problem: Fragmented Healthcare

Before TeleCure, the patient journey was broken:

  • Patients had to use separate apps for booking, paying, and pharmacy delivery.
  • Doctors lacked a unified tool for e-prescriptions and video calls.
  • Pharmacies had no automated onboarding to receive digital scripts.
  • Nurses had no interface for post-discharge "care at home" follow-ups.

The client needed a single source of truth – from appointment booking to IoT-enabled remote monitoring.

System Architecture & Feature Implementation

We designed a modular microservices architecture hosted on a HIPAA/PDPA-compliant cloud. Here is how we built each requested feature:

A. Appointment Booking & Management

Solution:

A redis cache back-end has been implemented to store available appointment slots.

User Flow:

Patients select the physician’s specialty → View available physician calendar slots (color-coded) → Book appointments with physician on Google/Apple calendar.

Admin Panel:

Performs drag/drop scheduling, send reminders via WhatsApp/SMS, and manage appointments of individuals in waiting list.

B. Video Call & Prescription Writing

  • Solution: Integrated WebRTC-based video SDK (low latency, even on 3G networks in rural Malaysia).
  • In-Call Tools: Doctors have a floating widget to write e-prescriptions (digitally signed) and medical certificates (e-MC) during the call. The patient receives a PDF instantly in the chat.

C. Pharmacy Onboarding & E-Commerce Section

  • Challenge: Onboarding physical pharmacies to a digital storefront.
  • Solution: A "Vendor Portal" for pharmacies to manage inventory, pricing, and delivery zones.
  • E-commerce Engine: Built a cart system supporting prescription uploads. If a user orders a prescription drug, the system flags it for doctor approval before payment.

D. Delivery & Payments

  • Integration: Multiple gateways (Stripe, Touch 'n Go, and local bank transfers).
  • Logistics API: Connected to third-party couriers (e.g., Lalamove, DHL) for real-time tracking. For urgent meds, we built a "Drone-ready" flag for future IoT expansion.

E. Doctor Onboarding & EMR for Patients

  • Doctor Panel: Uploaded license verification (MCR numbers) + KYC. After approval, doctors get a dashboard of past EMRs.
  • Patient EMR: A unified health timeline. Lab reports (uploaded as PDFs or images) are OCR-scanned and tagged. All data is encrypted at rest (AES-256). The EMR is PDPA-compliant, allowing patients to share access with a new doctor via a one-time link.

F. Nurse Module (Get for Care)

  • A unique post-discharge care option for patients that allows the attending physician to assign a ‘care plan’ to a nurse.
  • Nurse App: The nurse has a checklist of items, including vitals monitoring and wound care schedule. The nurse logs updates into the application, which are synced back to the physician’s EMR dashboard and provide data to support a reduction in hospital readmission rates of 18% based on the pilot program conducted by the customer on this module.

G. Integration of AI (the "Intellgience" Layer)

Three AI agents are built.

1. A Symptom Checker (NLP).

An AI chatbot will be developed so that a user can ask it questions about their symptoms, and the chatbot will be programmed to recommend which type of specialist (general, dermatology, or mental health) they should see prior to booking an appointment with that specialist.

2. A Prescription Refill AI

The AI will identify those medications that are routinely refilled by a patient. The AI will generate an electronic request for the physician to refill the patient’s medications prior to the patient coming into the office.

3. Voice to EMR

During a video-call, the AI listens to the call and creates a SOAP note (with the consent of the doctor), which saves two minutes of the doctor's time for each consult.

H. IoT Integration

  • Hardware: Connected Bluetooth bp cuffs and glucose monitors.
  • How It Works: Patient pairs device to the app → readings auto-upload to EMR. If BP > 140/90, the system triggers a "Red Alert" notification to the assigned nurse and doctor.

Execution & Overcoming Challenges

Challenge 1: Data Privacy (PDPA)

Action Taken:

We conducted a Data Protection Impact Assessment. All health data is stored on Malaysian servers. Storage of all health records has been located within Malaysia. We implemented RBAC to limit access/visibility to personal details; therefore, a nurse cannot access payment information and a delivery rider cannot view the patient diagnosis.

Challenge 2: Pharmacy Onboarding Speed

Action:

We created an Excel template to allow pharmacies to bulk upload (i.e., 1000+) SKUs through our pharmacy onboarding API. To validate medicine registration numbers, we used an automated script that cross references numbers in the NPRA database.

Challenge 3: Real-time IoT Sync

Action:

To ensure that through the app, IoT devices are synchronized in real-time with phone data, we utilized the MQTT Protocol to achieve a lightweight solution for syncing data from Bluetooth devices when the app was in the background.
